LENGTHENING OF THE SHORT-LIP DEFORMITY AFTER TRIANGULAR FLAP TECHNIQUE

Abstract

|
The most important advantage of Millard¢¥s operation is to preserve the philtrum unit and to lengthen the cleft side sufficiently. But some surgeons favor the triangular flap repair and it is caused by the surgeon¢¥s familiarity of that method. Anyway, plastic surgeons have encountered the short-lip deformity after the cleft lip repair.
There have been alternative methods for lengthening the lip following a triangular flap, such as, to design isoscele triangular flap and rotate into previous lower triangular flap, to make a new Millard¢¥s rotation advanement flap in upper triangular flap and rotate into the nostril sill or alar rim portion, and to use a Z-plasty on the nostril sill or white-roil etc.
In this paper, we propose a new technique to lengthen the short-lip deformity by positioning the correct lateral mark on Cupid¢¥s bow and by augmentation of the nostril sill with the scar flap. At once, it must be considered that alar deformity also affect the short-lip deformity and so we can remove the pseudo short-lip effect with the correction of alar hooding. First of all, it is important to consider the associated lip and nose deformity prior to simple diagnosis of short lip deformity.
